Heart to Home Meals is excited to announce that we’ve recently launched a program that provides next day delivery for discharge patients. Any senior within our Local Delivery Service Area who has been discharged from a hospital or rehabilitation facility is eligible for this program.

Heart to Home Meals will be able to provide next day, free delivery to anybody within our local service area. This service is available Monday through Friday from 9am to 5pm. The order must be placed prior to 2pm the day before delivery.

Anyone can place an order, whether it’s the customer, a family member, a caregiver, or a case manager. We accept credit card payment over the phone, or cash or check to the driver upon delivery.

Seniors who participate in this program will receive a two-hour delivery window. During delivery, drivers will happily bring the food into the senior’s home, even placing the meals in the freezer if needed. The senior can heat the meals in their oven or microwave.

We’ve partnered with several Aging Service Access Points (ASAPs), Home Care Providers, Rehabilitation Facilities, and Hospitals to spread the news about this program. Heart to Home Meals is proud to provide seniors this service, ensuring that recently discharged seniors have access to nutritious meals.
